Western medicine treatment

⑴Hemostasis

① Progesterone drugs: progesterone 20 mg/day, intramuscular injection for 3 days; Angong progesterone 8-16 mg/day, Funing tablets 4-8 mg/day or Fukang tablets 2.5-5.0 mg/day, for 5 days, orally. For those with prolonged amenorrhea and thick endometrium, testosterone propionate can be used simultaneously, 25 mg/day for young people and 50 mg/day for older people, intramuscularly for 3-5 days. Suitable for patients with hemoglobin greater than 6 grams and menopausal functional uterine bleeding.

② Estrogen preparations: diethylstilbestrol 2 mg, once every 6-8 hours. Reduce the dose by 1/3 3 days after the bleeding stops, and maintain the dose at 0.5 mg/day until 20 days after the bleeding stops. Or use 2n estradiol benzoate, injected intramuscularly once every 6-8 hours, with the same tapering method as above, and switch to oral diethylstilbestrol after the bleeding stops. It is suitable for adolescent patients with functional uterine bleeding who have acute massive bleeding and obvious anemia.

③ Other hemostatic drugs: such as Anluoxue, hemostatic min, hemostatic aromatic acid, vitamin K, etc., but the effect is not ideal, so it is only used as an auxiliary measure for hemostasis.

⑵ Adjust the menstrual cycle (apply after bleeding stops).

① Artificial cycle treatment (see oligomenorrhea).

② Estrogen-progestin combined therapy: generally oral contraceptives are used, and the method of administration is the same as contraception. It is suitable for people of childbearing age who have no desire to have children.

③ Progestin treatment: intramuscular injection of progesterone 20 mg/day for 3 days 1-2 days before the next bleeding or after the bleeding occurs, or oral administration of progesterone acetate, fumo tablets, etc. Used for menopausal functional uterine bleeding, for 3 consecutive cycles.

⑶ Restoring ovulatory function is the fundamental measure for treating pentoovulatory dysfunctional uterine bleeding during adolescence and reproductive period.

① Clomiphene (see frequent menstruation).

② Tamoxifen (see frequent menstruation).

③Human chorionic gonadotropin: on the 16th to 18th day of the menstrual cycle, 5000-10000 IU each time, intramuscularly once every other day, for a total of 2 times.

④ Gonadotropin-releasing hormone: Continuous pulse administration in the middle of the menstrual period, 5 mg per day for 3 days.

⑷ Induce amenorrhea: For menopausal and elderly women, use methyltestosterone 5-10 mg/day or cottonyphenol drugs to inhibit the endometrium, causing menstruation to gradually decrease until amenorrhea occurs.

⑸Hysterectomy: used for critically ill patients who have not responded to conservative treatment and do not need to have children.

Traditional Chinese Medicine Treatment

Basic treatment

⑴ Empirical

Treatment method: promote qi circulation, dispel cold, unblock menstruation and relieve pain. The main points are the Taiyin meridian and Ren meridian of the foot.

Main acupoints: Sanyinjiao, Zhongji, Ciliao

For patients with cold stagnation, add Guilai and Diji; for patients with qi stagnation, add Taichong; for patients with abdominal distension, add Tianshu and Qixue; for patients with flank pain, add Yanglingquan and Guangming; for patients with chest tightness, add Neiguan.

Use the needle-draining method, and moxibustion can be used for severe cold pathogens.

Fang Yi: Sanyinjiao is the meeting point of the three yin meridians of the foot, which can dredge the meridians and relieve pain. Zhongji is an acupuncture point on the Ren meridian, which can regulate the Qi of Chong and Ren meridians, dispel cold and promote the circulation of Qi. Ci Liao is an empirical acupuncture point for treating dysmenorrhea.

⑵ Deficiency syndrome

The treatment method is to regulate qi and blood, warm and nourish Chong and Ren meridians. The main points are the Taiyin and Yangming meridians of the foot.

Main acupoints: Sanyinjiao, Zusanli, Qihai

For those with deficiency of qi and blood, add Pishu and Weishu; for those with liver and kidney deficiency, add Taixi, Ganshu and Shenshu; for those with dizziness and tinnitus, add Xuanzhong.

The needle-needling method can be combined with moxibustion.

Formula meaning: Sanyinjiao is the meeting point of the liver, spleen and kidney meridians. It can strengthen the spleen and replenish Qi, regulate the liver and kidney, replenish the essence and blood of the liver, spleen and kidney, nourish the uterine vessels, and regulate the Chong and Ren meridians. Zusanli replenishes qi and blood. Qihai is a Ren meridian point that can warm the lower abdomen and nourish the Chong and Ren meridians.
