For people with high blood pressure, will the use of sartan drugs be more effective in lowering low blood pressure?

A friend with high blood pressure complained to Huazi that he was taking nifedipine sustained-release tablets, and his high blood pressure was well controlled, but his low blood pressure was too difficult to lower. He asked Huazi if there was any medicine that could control low blood pressure. Huazi told him that all antihypertensive drugs can lower both high and low blood pressure, and there is no drug that can control low blood pressure alone.

However, from a pharmacological point of view, if the low blood pressure is elevated, you can try adding a sartan drug, but the drug adjustment should be made under the guidance of a doctor.

1. Low pressure is produced by the aortic system

When the heart contracts, the blood that is discharged first enters the large arteries, and then gradually flows to the small arteries and capillaries. However, the blood discharged by the heart cannot flow to the small blood vessels all at once, so it can only be stored in the large arteries. When the heart relaxes, the large arteries elastically shrink, pushing the blood to flow to the peripheral small arteries.

That is to say, the aorta system is like a "balloon", which will exert pressure on the blood when it deflates. The lowest value of this pressure is the diastolic pressure, commonly known as low pressure.

There are two main factors that affect low pressure. One factor is the elasticity of the large arteries. The greater the elasticity (this is a good thing), the higher the low pressure. The other factor is the resistance of the peripheral arteries. The greater the resistance, the higher the low pressure.

2. Increased peripheral arterial resistance is the main cause

People with elevated systolic blood pressure mainly have increased peripheral arterial resistance. A common factor is increased sympathetic nerve tension, which leads to increased release of adrenaline and norepinephrine, causing peripheral arterial constriction.

There are many reasons for the increase in sympathetic nervous system tension, such as excessive pressure from work and life; or long-term negative emotions such as anxiety, depression, tension, and fear; or obesity and lack of exercise; or irregular work and rest, often staying up late, etc., which are all common factors.

The elasticity of large arteries in young people is normal, so the common characteristic of hypertension is normal high blood pressure and only a simple increase in low blood pressure.

3. Sartan drugs can be used to control low blood pressure

Sartans are first-line antihypertensive drugs, and are classified as angiotensin 2 receptor (AT receptor) blockers in pharmacology. Their mechanism of action is to block angiotensin 2 receptors on blood vessels to prevent them from binding to angiotensin 2, thereby relaxing blood vessels (both arteries and veins) and lowering blood pressure.

Sartan drugs can also block AT receptors on the presynaptic membrane of nerves, reduce the release of norepinephrine, reduce sympathetic nerve tension, restore the sensitivity of the carotid sinus (baroreceptors), and also promote lowering of blood pressure.

Therefore, sartan drugs have a better control effect on low blood pressure.

4. Most people need multiple drugs

Many people have told Huazi that when treating high blood pressure, taking only one drug is not very effective, and increasing the dosage will increase adverse reactions. Therefore, about 70% of patients with high blood pressure need to use two or more antihypertensive drugs in combination to control their blood pressure.

Sartan drugs are often used in combination with dihydropyridine drugs. While increasing the effect of controlling blood pressure, they can also reduce the side effects of dihydropyridine drugs causing lower limb edema.

If the heart rate exceeds 80 while the low blood pressure is elevated, a rolodex can also be used to lower the heart rate while controlling blood pressure. A rolodex can also be used in combination with a dihydrotestosterone.

To sum up, from the perspective of pharmacological mechanism, sartan drugs have a better effect on low blood pressure control. However, blood pressure cannot be reduced by drugs alone. While taking drugs, you should also follow a healthy diet with low salt, low fat and low sugar, and lose weight, increase exercise and reduce emotional stress.
