1. What is breast fibroid?

This disease is a common breast tumor in women, with a peak age of 20-25 years old, followed by 15-20 years old and 25-30 years old. The cause is the abnormally increased sensitivity of mammary lobular cells to estrogen, and the abnormal quality and quantity of receptors contained in fibroblasts. Estrogen is the stimulating factor for the occurrence of this disease, so fibroids occur during the ovarian function period. Its internal tissue structure is mainly composed of proliferative mammary lobule cells, epithelial ducts, fibrous tissue, adipose tissue and abnormal capillaries. Clinical manifestations: It is common in the upper outer quadrant of the breast, either single or multiple, with a texture as hard and elastic as elephant skin, a smooth surface, and is easy to push. The tumor grows slowly. Most patients have no symptoms, some feel local pain, and the swelling symptoms are slightly more severe during the menstrual period, but it has no effect on the size of the tumor.

2. Which treatment method should be chosen for breast fibroids?

Major medical materials and medical textbooks at home and abroad clearly point out that the only effective treatment for breast fibroids is surgical removal of the tumor. There are two methods for surgical removal of breast fibroids: one is traditional surgical removal of the tumor; the other is to use modern HIFU to perform in situ thermal removal of the tumor without surgery.

(1) Traditional surgical operation: It will leave surgical scars, affecting the overall integrity and beauty of the breast. It is difficult to remove multiple breast fibroids, smaller breast fibroids are not easy to detect and remove, and deep tumors are difficult to remove and the damaged area is large.

(2) Modern minimally invasive surgery such as Mammotome: The so-called "minimally invasive" surgery is to use a machine-assisted rotary cutter such as Mammotome to remove the tumor. The advantage is that the incision is small (except for scar tissue), only 0.3-0.5cm, which basically meets the beauty requirements of young women.

3. Comparison and decision-making between traditional open surgery and modern minimally invasive surgery:

1. Open surgery can be performed in one operation, while rotary cutting surgery requires multiple operations;

2. Open surgery can completely remove the tumor, while rotary cutting requires multiple mincing and removal;

3. Open surgery only removes the tumor under direct vision, but the rotary cutting knife will inevitably damage the surrounding normal glands;

4. Open surgery can be performed with regular excision, while rotary cutting can be performed with one cut on the left and one cut on the right for irregular excision;

5. Open surgery can ensure the removal of the entire tumor (left picture), which can ensure no residue. The rotary cutting knife can remove the tumor in stages (right picture); local bleeding; especially repeated rotary cutting, in order to ensure no residue, the trauma may be greater, so the appropriate tumor size is the key factor;

6. In addition, if the tumor is superficial, close to the skin, or located in the areola area, it may not be suitable for minimally invasive surgery.

7. Open surgery can effectively stop bleeding under direct vision, while excision surgery has a high incidence of postoperative hematoma. Therefore, although the incision is small, the trauma of the excision surgery may be greater than that of open surgery, and it is difficult to achieve "minimally invasive"; it is just a "micro entry" with a small incision.

Of course, if it is a clear fibroadenoma of 1.0-2.0cm, the ultrasound shows it clearly, and you are very concerned about the size of the incision; especially if you meet a skilled and experienced doctor; you can consider doing Mammotome atherectomy to achieve the best cosmetic effect. In addition, for multiple fibroadenomas, Mammotome rotary resection can be chosen to avoid excessive incisions.

The incision for open surgery is usually determined based on the size and location of the tumor. Doctors will usually make the incision in hidden places such as the areola or the outer or lower edge of the breast, but specific problems must be analyzed specifically. In addition, cosmetic suture techniques such as buried sutures are usually used. However, if the patient has a scar-prone constitution, both minimally invasive and open surgery will leave scars.

4. Will minimally invasive treatment affect breastfeeding when taking care of children in the future?

Because breast fibroids are more common in young women, with the peak age being 20-25 years old, who are generally not yet married and have children, it is also a common concern. Open surgery may affect the breastfeeding function of the affected side, but minimally invasive surgery will not affect breastfeeding in the future. So far, most of the patients with breast fibroids we treated have already given birth to children without any impact.
