Diagnosis of Corpus Luteum Rupture: Because its clinical manifestations are not typical, the main clinical symptoms are sudden onset of abdominal pain with varying degrees of pain. Some patients have varying degrees of anal distension. In severe cases, massive bleeding and even shock may occur, endangering life and requiring urgent surgery. The onset is acute, and there is currently no special examination method to make a clear diagnosis. The misdiagnosis rate is high and it is easily confused with acute abdomen such as ectopic pregnancy, acute pelvic inflammatory disease, acute appendicitis, ureteral stones, and ovarian tumor pedicle torsion. Main diagnostic basis: 1. The patient has no history of amenorrhea, and the onset of the disease often occurs between two menstrual periods or before menstruation; there is a history of onset after sexual intercourse. 2. The onset is sudden, with sudden severe pain in the lower abdomen, which becomes continuous pain after a short time, and may gradually ease or become severe again. A small number of patients may experience nausea and vomiting. There is generally no vaginal bleeding (external bleeding). Severe internal bleeding may cause shock and rectal irritation symptoms. 3. Abdominal examination: obvious tenderness and rebound tenderness. If there is a lot of internal bleeding, there will be mobile dullness on percussion. 4. Vaginal examination: The uterus is of normal size, the posterior fornix is tender, and a soft mass with unclear boundaries and tenderness can be felt in the adnexa. 5. White blood cell count and classification, neutrophil percentage increased, red blood cells and hemoglobin decreased. Corpus luteum rupture often occurs on the right side, but lacks typical symptoms and should be differentiated from acute appendicitis, ruptured fallopian tube pregnancy or miscarriage. During the development of the corpus luteum, the small blood vessels on the surface of the ovary may be damaged by chance, causing internal bleeding in the corpus luteum, leading to increased internal pressure and rupture, which is called ovarian corpus luteum rupture. Severe cases can cause massive intra-abdominal bleeding, which is ovarian rupture. |
