Menstruation is a very normal physiological manifestation in women. Girls who enter puberty will have menstruation every month. There will be many symptoms when menstruation occurs, including the discharge of follicles. However, it also has some abnormal manifestations, such as menstruation without follicles. So what is the reason for menstruation without follicles?

There are two types of menstruation: ovulatory menstruation and anovulatory menstruation. Anovulatory menstruation occurs without ovulation.

Follicular development and ovulation are regulated by the hypothalamus-pituitary-ovarian gonadal axis, so any abnormality in the gonadal axis can cause ovulation disorders. The main reasons for non-ovulation during normal menstruation are as follows:

Without ovulation, there will be no corpus luteum formation, and there will be no normal secretion of progesterone. Some women have regular uterine bleeding, but it is not true menstruation, but anovulatory menstruation. Without ovulation, there will be no corpus luteum formation, and there will be no normal secretion of progesterone, which will lead to female infertility.

After ovulation, the follicle wall atrophies and the granulosa cells enlarge to form the corpus luteum. The corpus luteum secretes estrogen and progesterone. When the levels of estrogen and progesterone drop, the endometrial blood vessels contract, causing ischemia, necrosis, shedding, and bleeding, resulting in menstruation.

The peak of estrogen secretion cannot be reached, resulting in the ovaries being unable to ovulate.

Because the corpus luteum secretes a large amount of progesterone and estrogen, which in turn inhibits the hypothalamus, causing the luteinizing hormone and follicle-stimulating hormone to decrease, the corpus luteum to atrophy, and the secretion of progesterone and estrogen to decrease. The endometrium cannot get the support of estrogen and progesterone, so necrosis, shedding, and bleeding will occur, and menstruation will occur.

In addition, if the follicle only develops to a certain extent and does not reach a diameter of 18 mm, it means that the follicle is not mature. Although it can secrete a certain amount of estrogen, it cannot reach the peak of estrogen secretion, resulting in the inability of the ovary to ovulate.

If you have menstruation without ovulation, you can use a combination of Chinese and Western medicine for treatment. According to the special physiological phenomena of the female menstrual cycle, medication can be used in stages, using the combined effects of Chinese and Western medicines to regulate the body's endocrine system, promote the development and maturation of follicles, and achieve the purpose of fertility.
