Cervical laceration, like uterine rupture, is more likely to occur when the force of delivery is too strong. When the cervical canal resistance is too strong in older primiparas, when the fetus is too large, or when the fetus is in an abnormal position, the cervical canal may be injured by strong force. After delivery, the uterus contracts well, there are no wounds in the vagina and vulva, but bleeding is always continuous, which can also be considered as cervical laceration.

Cervical lacerations often occur during childbirth. If the laceration is shallow and there is no bleeding at the time, it often goes undetected. Only when the laceration is deep and the bleeding is heavy can it be discovered through examination. Once cervical laceration is found, it should be sutured immediately. When suturing, if the sutures are neatly aligned and the healing is good, there will be no obvious signs of laceration after the cervix has completely returned to normal. If the laceration is deep, not sutured at the time, or the closure is not good, and the healing is poor, after the cervix recovers, you can see the cervical opening split open, appearing as a large horizontal crack or split into several petals, much like a cleft lip. This is called a chronic cervical laceration. Due to cervical laceration, the cervical canal mucosa often turns outward and is immersed in acidic vaginal fluid all day, which can easily cause infection. After infection, the amount of leucorrhea increases, and sometimes it becomes pus-like or bloody. Contact bleeding often occurs when touching it with fingers during sexual intercourse or internal examination. If the infection spreads inward, the parauterine tissue and main ligaments will also thicken due to inflammatory infiltration, and patients will often feel pain in the lumbar region. The incidence of cervical cancer in people with old cervical lacerations is slightly higher than that in normal people.

The placenta is intact, the uterus contracts well but there is persistent vaginal bleeding (may be bright red with blood clots). Examination of the perineum shows no lacerations, so cervical lacerations may be considered.

After the third stage of labor, if the uterus contracts well but vaginal bleeding continues, the vagina must be dilated, the cervix must be clamped with two sponge forceps, and an examination must be performed around the cervix. If there is a laceration, use sponge forceps to clamp both sides of the laceration and suture it intermittently with No. 1 chromic gut thread.

3. Any intravaginal or intrauterine operation, such as internal version, decapitation, craniotomy and artificial placental removal, should be performed gently and correctly, avoiding violence.
