Due to insufficient estrogen secretion before ovulation, the corpus luteum may become poorly developed and atrophy prematurely. When the corpus luteum is incompletely developed, its secretory function is poor, resulting in insufficient progesterone secretion. The clinical manifestations are regular menstrual cycles, but the cycles are shortened, or there is a small amount of bleeding a few days before the menstrual period, and the amount of menstrual blood may not change. Premenstrual endometrial biopsy may reveal poor or uneven glandular secretion. Interstitial edema was not obvious. The basal body temperature is biphasic, but rises slowly, and the luteal phase is shorter than normal, generally around 10 days. Due to insufficient gestational period, infertility or early miscarriage often occurs.

3. Clinical significance

1. Combined detection of serum LH and FSH: For women, it is mainly used to distinguish between primary (ovarian) amenorrhea and secondary (pituitary) amenorrhea, the latter of which shows decreased serum LH. For men, it is mainly used to differentiate between primary and secondary testicular insufficiency.

2. Serum LH testing can identify true or false precocious puberty in prepubertal children.

3. During the menstrual cycle, once the LH peak appears, it indicates ovulation in the ovaries within 24 to 36 hours. Therefore, monitoring the serum LH peak in the middle of the menstrual cycle can determine the best time for conception.

4. Increased serum LH is seen in polycystic ovary syndrome (persistent anovulation and excess androgen, etc.), Turner syndrome, primary hypogonadism, premature ovarian failure, after oophorectomy, and in menopausal syndrome or menopausal women.

5. LH may also decrease after long-term use of contraceptives and hormone replacement therapy.
