Uterine septum is a manifestation of incomplete cavitation during the congenital development of the uterus. It is characterized by a ridge of connective tissue at the fundus of the uterus protruding into the uterine cavity, deep uterine horns, and a relatively small uterine cavity volume. Patients with polycystic ovary syndrome often have septum. Through vaginal four-dimensional color ultrasound coronal imaging and dynamic digital hysterosalpingography, it can be seen that the fundus of the uterus is sunken in the horizontal position. The diagnosis is confirmed by hysteroscopy.

A large mediastinum can lead to infertility, just like placing a "meat ring" in the uterine cavity, which is equivalent to placing an IUD; a medium mediastinum can lead to ectopic pregnancy, possibly because the mediastinum in the uterine cavity affects implantation and the fertilized egg wanders outside the uterus; a small mediastinum can lead to habitual miscarriage, possibly because the fertilized egg implants on the mediastinum, and because of its poor blood supply, the fetus stops growing.

Pathogenesis

Uterine septum is caused by obstruction of the fusion of the bilateral mesonephric ducts during congenital development. The incidence of uterine septum and bicornuate uterus is 5%. Uterine septum changes the symmetry of the uterine cavity and interferes with normal reproductive function. Even if a woman with a septate uterus becomes pregnant, the incidence of fetal intrauterine growth retardation and intrauterine fetal death is high.

When the embryo develops normally, at 19 to 20 weeks of gestation, the intrauterine septum formed by the fusion of the two Mullerian ducts will be completely absorbed and disappear. If the absorption process is hindered by internal and external factors, it may cause uterine septum of varying degrees. In recent years, cell apoptosis is considered to be one of the mechanisms of uterine septum absorption during the embryonic period. The results of monoclonal antibody and immunohistochemical analysis showed that no apoptosis-regulating protein (Bcl-2) was found in the septate uterus. Therefore, the absence of Bcl-2 leads to malabsorption of the uterine septum during the embryonic period.
