Whether acute or chronic endometritis is accompanied by cervical canal obstruction, so that the inflammatory secretions in the uterine cavity cannot flow out or the drainage is poor, intrauterine pyometra can be formed, but it may be caused by ascending infection of cervical inflammation. The main causes of uterine effusion are: uterine bleeding; endometritis; adhesion and blockage of the cervical canal; and genital malformations.

When uterine effusion occurs, the patient may experience lower abdominal pain, increased vaginal discharge and bloody secretions. If the above situation occurs, you should go to a regular hospital for treatment, undergo hysteroscopy and uterine curettage, and then carry out appropriate treatment for uterine fluid after the cause is determined. Artificial abortion can also cause uterine fluid accumulation, which can be checked by B-ultrasound.

The most common cause is inflammation of the fallopian tubes or pelvic peritoneum. When the fallopian tubes become inflamed due to gonococcal infection, tuberculosis infection, chlamydia infection, mycoplasma infection or other bacterial infection, the epithelial tissue of the fallopian tube endometrium can be destroyed, causing the fallopian tube to be blocked. The inflammation can destroy the fallopian tube mucosa and form scars, causing narrowing or blockage of the tube lumen.

1. The main symptom is lower abdominal pain, which may be accompanied by systemic symptoms, fever, and increased white blood cell count.

2. Intrauterine pyometra that gradually forms due to chronic endometritis may have no obvious symptoms.

3. Gynecological examination shows that the uterus is enlarged, soft and tender. The parametrial connective tissue may be significantly thickened, and there may be an inflammatory mass in the adnexal area.

Testing

1. Use a uterine cavity probe to insert the uterine cavity. If pus flows out, the diagnosis can be confirmed.

2. Bacterial culture and drug sensitivity test of pus. If necessary, scrape the endometrial tissue for pathological examination to determine whether there is malignant lesion.

Surgical procedures

1. Body position: lithotomy position.

2. Exposure of the cervix

After disinfection of the vagina and cervix, place a vaginal speculum and fix the cervix with a cervical clamp.

3. Exploring the uterus and draining the fluid

Use a probe to detect the depth of the uterus in the direction of uterine inclination. After insertion, use a cervical dilator to expand the uterine cavity until the accumulated fluid can flow out smoothly. Perform diagnostic curettage when necessary.
