Patients who have developed uterine prolapse should adopt a combination of traditional Chinese and Western medicine, as well as comprehensive measures combining treatment, nutrition, and rest. In terms of treatment methods, it can be divided into: the use of pessary, oral Chinese medicine, acupuncture, fumigation and other non-surgical treatments and surgical repair. Because the operation will have a certain impact on the subsequent vaginal delivery, it is only suitable for severe cases and women who no longer want to have children.

1. Pessary therapy

(1) Pessary and its function: Pessary has been used to treat uterine prolapse for a long time. Through continuous improvements in the prevention and treatment of uterine prolapse in various provinces and cities, good results have been achieved. This method is simple and easy to perform, and patients can master it by themselves. It is very popular among rural patients and can be used for all degrees of uterine prolapse. There are many types of pessaries, and the most commonly used pessary is the plastic mushroom type. The trays are divided into three sizes: large, medium and small (the diameter or horizontal diameter is 6, 5 and 4 cm respectively). Trays are divided into two types: round and oval. The most commonly used size is medium. The handle is about 5cm long and bends forward to fit the curvature of the vagina.

Pessary treatment is to use the pubococcygeus muscle bundle of the levator ani muscle to support the uterine tray in the vaginal vault, prevent the cervix from descending, maintain the cervix at the level of the sciatic muscles, and keep the handle level with the vaginal opening. For mild cases, no additional support is needed. If the vagina is too loose, a menstrual belt must be used to support the handle, or a belt or plastic rope must be passed through the upper end of the handle and fixed to the belt front and back to prevent it from falling out.

(ii) Selection of pessary size: It is best to use a pessary that is slightly larger than the genital (pubococcygeus muscle) fissure. Generally, the maximum horizontal diameter of the fissure is 4 cm, so medium-sized pessaries are often used. After a period of time, the pubococcygeus muscle gradually recovers its elasticity, the tissue edema disappears, the weight is reduced, and the uterus will no longer prolapse after the prolapsed part is restored.

(III) Usage time: Generally put it in the morning before work, take it out at night and wash it. It is best not to use during menstruation. The surface of the plastic tray is smooth, it is not easy to deteriorate when exposed to acids and alkalis, and it has little irritation to tissues. After wearing the support, the symptoms disappear and the patient can participate in various activities without pain. The effect will be better if combined with acupuncture and Chinese medicine treatment.

Patients with severe uterine prolapse and excessive vaginal relaxation should not use a support.

(IV) Tray placement method: Wash your hands first, the patient lies half-lying on the bed or squatting on the ground, with legs apart, holding the tray handle with one hand, and move the tray handle close to the perineum and anus, so that the tray is in a horizontal position and enters the vagina. If it is an oval tray, the narrow end of the tray must be inserted into the vaginal opening first, and the handle of the tray is gradually rotated upward to allow the entire tray to enter the vagina. The handle is then turned to bend forward.

(V) Removing the tray: Before going to bed, pinch the handle of the tray with your fingers, pull it outwards while pressing it toward the anus, and the tray will slide out of the vagina. The tray can be washed with warm water, wiped dry, and wrapped for reuse.

(VI) Precautions for using pessary:

1. Patients must be taught to put in and take out the splint by themselves, and be advised to take it out and clean it every night. Elderly people who do not have sex should also take out the pessary every 2 to 3 days, clean it, and then put it back in to avoid leaving it in for too long and causing vaginal inflammation. If it is rubbed with the handle or tray, it may cause ulcers on the vaginal wall and form a vaginal scar ring, which may cause the pessary to become incarcerated and even cause compressive necrosis of the tray, penetrating the bladder or rectum to form urinary fistula or fecal fistula.

Second degree uterine prolapse is indeed a big problem for many women, because once the uterus prolapses, not only will various diseases appear in the body, but even sick children will become more difficult. Therefore, when a woman has second degree uterine prolapse, she should pay great attention to it, including all aspects.
