Does an incomplete septate uterus require surgical treatment?

Women with incomplete septate uterus usually feel desperate, because some women think that if they do not have surgery, they are likely to become infertile, but if they have surgery, they are afraid of damaging the intact part of the uterus. So in this situation, many women will hesitate and don’t know whether they should have surgery. But in fact, if you are well prepared before the surgery, the surgery will still be very successful.

1. General Preparation

In addition to routine preoperative examinations and necessary psychological preparation, all patients should undergo ultrasound examination, laparoscopy, hysterosalpingography (HSG), or pelvic and uterine double angiography to confirm the diagnosis of congenital Müllerian duct malformation with uterine septum, and estimate the width and length of the septum in advance.

2. The best time for surgery is 3 to 7 days after the menstrual period ends. If the surgery is performed in the second half of the menstrual cycle, the pelvic congestion and incomplete healing of the intrauterine wound may easily lead to an increase in the amount of blood in the first menstrual period after the operation.

Surgery can also be performed after treatment with an endometrial growth inhibitor.

Subtotal hysterectomy is the earliest used hysterectomy procedure, which is simple to operate and has few complications. As early as before the 1850s, its usage rate accounted for 95% of all hysterectomies. Later, in order to avoid the occurrence of cervical stump cancer, total hysterectomy gradually replaced the monopoly of subtotal hysterectomy.

Now, with the improvement of cervical cancer screening methods, the reduction in the incidence of cervical stump cancer (0.11% ~ 0.14%), and the general improvement in people's demand for quality of life, subtotal hysterectomy has gradually received attention.

Bojahr et al. in Germany retrospectively analyzed 1,604 cases of laparoscopic subtotal hysterectomy (LSH) and believed that subtotal hysterectomy is easy to perform, has a low postoperative morbidity, causes little damage to the bladder and pelvic floor nerves, and can maintain good bladder function and sexual function after surgery. Therefore, subtotal hysterectomy is considered to be the best surgical procedure for hysterectomy.

After the occurrence of uterine septum, it must be treated with surgery. The main things to pay attention to before the operation are, and sexual intercourse must be prohibited for one month after the operation, so that the uterus can recover better. Also, try to eat more nourishing foods in your diet to help the recovery of the uterus.
