A 6-year-old boy died after circumcision surgery? Should circumcision be performed?

"My son is 5 years old now. Is it time for a circumcision?"

"My friend's child has had a circumcision, should my child also have one?" "It's vacation time, let's take our child to have a circumcision together?"

Every summer, "circumcision" becomes one of the "hot topics" in parenting groups.

Recently, the topic # 6-year-old boy died 10 days after being discharged from hospital after circumcision surgery # became a hot search

It has attracted more attention from parents

On July 24, in Fuzhou, Fujian, a 6-year-old boy developed low-grade fever and other discomfort symptoms after undergoing circumcision surgery. He was sent to the hospital for emergency treatment on the evening of August 3, but unfortunately he was not rescued. The child's family questioned that the child showed abnormal indicators before the operation and the hospital did not fulfill its obligation to inform. The indicators were not rechecked before discharge after the operation, so the hospital has a great responsibility. "We hope that the hospital will apologize for this matter and give corresponding compensation." Mr. Jiang said. "The patient's current cause of death is unknown. There are many possibilities such as poisoning, trauma, acute disease attack, and delay by the patient. It has nothing to do with our hospital's previous diagnosis and treatment. It is recommended that the patient's family perform an autopsy to further clarify the cause of the patient's death." The hospital staff said. On the morning of the 7th, according to the Fuzhou Municipal Health Commission, the Medical Administration Department of the Health Commission has intervened in this matter. At the same time, the family also said that they would consider an autopsy to give the child justice.

So, should circumcision be done? When is it necessary?

And at what age is better?

In fact, 50% of boys have their foreskin retracted to their normal position before the age of 1, and 89% by the age of 3. The incidence of phimosis in boys aged 6 to 7 is 8%, and only 1% in boys aged 16 to 18.

How to determine foreskin abnormality?

Parents only need to turn over their children's foreskin. If the foreskin can be turned over to reveal the urethra and glans penis, it is foreskin that is too long; if the foreskin cannot be turned over to reveal the glans penis, it is phimosis. Generally speaking, children under 3 years old have phimosis. If it is still phimosis after 3 years old, it needs to be paid attention to. If there is no recurrent inflammation, it can be evaluated when the child is 5 or 6 years old.

Does foreskin that is too long or phimosis require surgery?

If the foreskin is too long, you only need to turn it up regularly and clean it with warm water, but do not force the foreskin up to clean the smegma, so as not to cause damage and increase the risk of infection. Most cases of foreskin or phimosis are physiological, and most of the cases can be improved automatically as the child grows up and enters puberty.

What? I have to masturbate my son?

In a sense, some phimosis that requires medical intervention is the result of parents' "inaction" - the only blame is that they did not teach their children scientific masturbation techniques from an early age.

Parents should help their sons masturbate every time they take a bath, starting when they are about 1 year old. Push upwards gently, exposing as much as possible each time. For most boys, the foreskin will be fully retracted one day.

The reason for choosing to wash it during a bath is that in a warm water environment, the foreskin is more elastic, and with the lubricating effect of water, it is easier to turn up.

I want to remind you that the purpose of turning it up is to clean it, and you should turn it down immediately after cleaning it. Otherwise, the foreskin will not be able to retract and will get stuck in the center of the penis, which will cause swelling. In addition, don't force it to be effective in one go. Not only will it hurt, but it may also cause the foreskin to be unable to recover and get stuck in the penis head, causing bruises.

Some people may say: It hurts to rub it, and the child won't let me turn over. Warm reminder: Parents who really can't do it can go to the pediatric urology department and ask the doctor to help rub it.

Under what circumstances is surgical treatment required?

If the child's foreskin is still too narrow and cannot be turned up after the age of 6, circumcision is required. In addition, if there is balanitis sicca (atrophic lichen sclerosus), phimosis with recurrent balanitis, phimosis with urinary dysfunction, scar phimosis, posterior urethral valve disease, or urinary system malformations combined with recurrent urinary system infections, surgical treatment is necessary. There is also a situation where the foreskin is too long and needs to be treated, that is, recurrent balanitis.

It should be noted that "hidden penis" cannot be circumcised. In layman's terms, hidden penis means that the penis is small in appearance, the foreskin is short, and the penis is retracted in the subcutaneous tissue, but the penis itself is normally developed. The foreskin opening is often narrow, and the foreskin cannot be turned up to expose the glans penis, which is often confused with phimosis.

When is the right time to have circumcision surgery?

The external genitalia of boys develop rapidly after entering puberty. It is recommended that circumcision surgery be performed in the early stages of puberty, and the best age is 6 to 7 years old. Of course, most of the patients who undergo surgery are children aged seven or eight, but there are also older children and adults. If it is simply foreskin that is too long, you can decide whether to have surgery after the puberty is over. If there are other special conditions or diseases, and the doctor determines that surgery is necessary, it is best to perform surgery in a timely manner even if the patient is young.
