Lower abdominal pain during menstrual bowel movement

It usually occurs before or after menstruation. The patient feels unbearable pain when excrement passes through the duodenum, but there is no feeling at other times. It is a typical symptom of endometriosis in the uterine duodenal fossa and around the duodenum. If a small amount of disordered endometrium reaches deep into the duodenal mucosa, there will be rectal bleeding during menstruation. Endometriosis closely surrounds the duodenum and causes stenosis, causing anal prolapse and obstruction, which is similar to cancer. So, what is the reason for lower abdominal pain when having a bowel movement during menstruation?

1. Menstrual pain

It is a common and prominent symptom, which is mostly secondary, that is, since the occurrence of endometriosis, the patient reports that there was no pain during the menarche in the past, but menstrual pain began to occur at a certain stage. It can occur before, during, and after menstruation. Some women have severe and unbearable menstrual pain and need to rest in bed or take medicine to relieve the pain. The pain often worsens during the menstrual cycle. Because the level of estrogen continues to rise, the disordered endometrium proliferates and swells. If it is affected by estrogen again, it will cause bleeding, irritation to local tissues, and even pain. If it is endogenous endometriosis, it can promote uterine muscle atrophy, and menstrual pain will inevitably be more obvious.

2. Infertility

Endometriosis patients are often accompanied by infertility. There is still controversy about the logical relationship between infertility and endometriosis. Pelvic endometriosis can often cause adhesions around the bilateral fallopian tubes, affecting the collection of oocytes or causing obstruction of the lumen. Or infertility may be caused by uterine and ovarian diseases affecting the normal development of ovulation. However, some people believe that long-term infertility and the absence of a menstrual period can lead to the chance of endometriosis, and once pregnancy occurs, the disordered endometrium is suppressed and shrinks.

3. Clinical symptoms of protrusion

Patients with endometriosis usually have an enlarged uterus, but rarely become pregnant beyond three months. It is common to see consistent enlargement, and some parts may look very similar to uterine fibroids. If the uterus is posterior, it is usually fixed by adhesions. In the uterine duodenal fossa, uterine sacral tendon or posterior wall of the cervix, one or two or a large number of small nodules, such as the size of mung beans or soybeans, can often be touched. Most of them have obvious dull pain, which is more obvious during rectal examination. This is very important. Dark purple internal bleeding spots or masses may be seen inadvertently in the posterior fornix of the vagina. If there are many lesions in the duodenum, a hard lump may be felt, which may even be misdiagnosed as rectal cancer.

In addition to gynecological diseases, there are many diseases that may be accompanied by lower abdominal pain, such as intestinal inflammation, acute cystitis, etc., which require careful and targeted physical examinations to be diagnosed. Therefore, if women experience lower abdominal pain during menstruation, they should go to the hospital for a physical examination as soon as possible and not take chances.
