Medical abortion against early pregnancy refers to achieving artificial abortion in the early stages of pregnancy without the need for surgery, but by means of injections or medication. The use of drugs to terminate pregnancy is the latest development in the past 20 years. Currently, the commonly used drugs are mifepristone (Ru 486) and prostaglandins. The former causes degeneration and necrosis of the uterine decidua and softening of the cervix, while the latter causes uterine contraction and promotes the expulsion of the embryo. Medical abortion is simple, effective, non-traumatic, and avoids the complications that may be caused by entering the uterine cavity. Currently used to terminate pregnancies up to 8 weeks. The complete abortion rate has reached 90%-95%. Suitable for pregnant women within 49 days without contraindications.

Secondary uterine contractions and abdominal pain occur, and the embryo is usually expelled within 6 hours, while a few are expelled within a week. If the feces cannot be discharged completely, the pregnancy will continue and an instrumental abortion will be required. During the abortion process, in addition to lower abdominal pain. Some patients experience vomiting, diarrhea and fever. The amount of bleeding after medical abortion is high, and the average bleeding time is 10-15 days. The body's resistance decreases after abortion, so more attention should be paid to personal hygiene.

Because of the wounds left on the endometrium, vaginal secretions increase, making it a breeding ground for bacterial infection and reproduction. Therefore, special attention should be paid to the cleanliness of the vulva, shower and clean the vulva in time, disinfect the toilet paper and change it frequently; avoid taking a bath in a tub within half a month, and change underwear frequently; and sexual intercourse is absolutely prohibited within one month to prevent bacterial infection.

In addition, contraception should be further strengthened. Under the guidance of a doctor, use appropriate contraceptive methods or sterilization measures to prevent pregnancy again. You should also pay attention to vaginal discharge after the operation. Generally, there will be red or light red discharge within a week, and the amount will gradually decrease without any obvious odor. If there is still bleeding or excessive secretions after a week, especially if the secretions have a foul odor and are accompanied by symptoms such as fever and abdominal pain, you should go to the hospital for examination and treatment in time.

Pharmacology: Mifepristone is an anti-progestin drug at the receptor level. It has the effects of terminating early pregnancy, preventing implantation, inducing menstruation and promoting cervical ripening. It competes with progesterone for receptors to antagonize progesterone and also has a certain binding force with glucocorticoid receptors. Mifepristone can significantly increase the sensitivity of the pregnant uterus to prostaglandins. Small doses of mifepristone combined sequentially with prostaglandins can achieve satisfactory results in terminating early pregnancy.
