1. Treatment principles

(1) Identify the cause and provide appropriate treatment.

(2) Combined internal and external treatments can slow down changes in skin lesions.

(3) Avoid local trauma and irritation, observe the condition carefully, and prevent malignant transformation.

(4) For those suspected of having leukoplakia or cancer, an early biopsy should be performed to confirm the diagnosis.

2. Systemic treatment

1. Senile vulvar atrophy and primary vulvar atrophy generally do not require special treatment. Those with obvious symptoms can take estrogen or B vitamins orally and receive symptomatic treatment.

2. The causes of atrophic lichen sclerosus should be eliminated, and those with diabetes, trichomoniasis or fungal vaginitis need treatment. Eat less spicy food, keep the vulva clean, avoid scrubbing with soap, scratching and using irritating drugs. Taking sedatives, desensitizers, diethylstilbestrol, chloroquine, and vitamin A and vitamin E may have certain therapeutic effects.

3. Local treatment

It is mainly used to relieve itching, reduce inflammation, moisturize the skin and improve local nutrition.

(1) For patients with vulvar pruritus and vulvitis, local antipruritic agents, antibiotic ointments, estrogen ointments, etc. are used for symptomatic treatment. Patients with concurrent vulvar infection are given anti-infection treatment.

(2) Atrophic lichen sclerosus:

① You can use Sophora flavescens and Artemisia capillaris lotion or other Chinese herbal decoctions for fumigation and washing.

② For patients with hyperplastic and mixed types, dexamethasone and hydrocortisone ointment can be used.

4. Surgery

Surgery may be considered for moderate to severe atypical hyperplasia, or ulcers and fissures that do not heal after non-surgical treatment.
