Endocervical canal - examination (1) Vaginal examination: Observe the position, shape, size of the external opening of the cervix, the amount and nature of secretions, and the presence of cervical erosion and neoplasms, and pay attention to the presence of cervical lifting pain and parauterine tenderness. At the same time, bacteriological examination of cervical mucus is performed. If necessary, do a scraping or colposcopy. (2) Probe examination: It is simple and practical. It can detect the direction and length of the cervical canal and its ratio to the cervix, as well as the presence of stenosis, adhesions, and the tightness of the internal cervical os. For those suspected of abnormalities, further angiography should be considered. (3) Contrast examination: Hysterosalpingography can not only detect uterine and fallopian tube lesions, but endocervical canalography is also extremely helpful in studying infertility. When oil is used and the contrast instrument does not affect the development of the cervical canal, the length and shape of the cervical canal can be well displayed. Endocervical growths, congenital malformations, stenosis, adhesions and uterine buckling can all be clearly diagnosed. Cervical fibroids, polyps, etc. appear as filling defects with smooth edges on cervical canal films; while the edges of cervical adhesions are mostly irregular serrated shapes. The cervix of a hypoplastic uterus is narrow and long, while the cervical canal of a tuberculous uterus is relatively wide and has brush-like changes. Angiography can also be used to measure the length and proportion of the cervical canal and uterine body. Angiography is often successful, especially for patients with genital dysplasia and cervical stenosis who have failed probe examinations. Endocervical canal - Treatment 1. Cervical atresia and stenotic cervix are mainly treated with uterine dilation. It is performed after the menstruation is over or before ovulation. First, a sterilized probe is used to probe into the uterine cavity, and then the cervical dilator is used to dilate from No. 3 to No. 7. Congenital cervical dysplasia can be treated simultaneously with artificial cycles or high-dose progesterone as pseudopregnancy therapy. After treatment of congenital cervical adhesions or stenosis with uterine dilation, a drainage tube is placed in the cervix to prevent re-adhesion. 2. Abnormal cervical position: There is no good treatment for this disease. You can try the following methods: ① Post-coital hip training: during or after intercourse, place two pillows on the woman's hips, so that her head is low and her hips are high. This can help the semen accumulate in the posterior fornix as much as possible, relatively raising the semen level, which is conducive to the cervix being immersed in semen; ② Prone position after sexual intercourse: The purpose of this method is to encourage semen to accumulate in the anterior fornix of the vagina after sexual intercourse, so that the upturned cervix can be easily immersed in semen; ③When the above methods are ineffective, intrauterine artificial insemination can be used. |
