Most people know that there are two types of abdominal incisions in cesarean section, transverse incision and vertical incision. The two types of incisions are in different locations, so their appearance, postoperative pain, and wound healing are all different. The vertical incision uses a vertical method, cutting vertically from the navel to the top of the toe bone. The incision is on the center line between the navel and the ischial tuberosity, and is about 10 cm long.

The transverse incision is to cut open the abdomen of the pregnant woman horizontally. The wound is usually 2 cm above the ischial tuberosity line and is about 10 cm wide.

Whether it is a transverse incision or a vertical incision, it is not that simple, because the method of laparotomy is to cut the lower abdomen first, and then cut the fat, fascia, abdominal muscles, retroperitoneum, uterine muscle layer in turn, then double layers, and finally cut the amniotic fluid cavity of the pregnant woman, after taking out the fetus and embryo, and then suture the above layers in turn, and finally suture the abdominal skin.

In other words, whether it is a horizontal section or a vertical section, it is just a wound method, which is the first cut to open the skin, and the other cutting levels and processes are the same.

In the past, doctors basically only made vertical incisions during operations. This was because vertical incisions were larger and it was very easy for doctors to remove the fetal head. There was no need to press lightly on the uterine fundus, which made accidents less likely to occur.

For doctors who lack experience and have average skills, vertical incision surgery has sufficient visual exposure and the incision can be increased depending on the situation, making it a better choice.

Also, if it is an emergency cesarean section, due to time constraints, vertical incisions are also recommended. Since the operation time is short, it can even be performed under local anesthesia in an emergency.

However, vertical incisions have two drawbacks that pregnant women cannot accept:

One is that the vertical scar on the abdomen is obvious. A long vertical scar is very ugly. It will be a nightmare for pregnant women with scar-prone constitution.

Another disadvantage of vertical incision is that after the operation, the pregnant woman will feel very painful when she wakes up, lies down, and stretches heavily. She also has to use a belly band to fix it, which is inconvenient.

Compared with vertical wounds, the advantages of transverse incisions are irreplaceable. The most critical thing is that the wound of a transverse incision is low and can be hidden directly under the women's underwear after healing. The horizontal and oblique lines of the wound are consistent with the skin texture and are not easy to be found. Second, after transverse incision surgery, pregnant women should stretch less when standing or lying down, and the pain after surgery should be relatively mild.

However, transverse incision requires a higher level of technical skills from the doctor, the operation is more time-consuming, and the requirements for anesthesia are also high.

In Western and European countries, transverse incisions are promoted instead of vertical incisions. In our delivery room, when we first started working, we still used the vertical incision method. Later, we learned advanced technology from abroad and started to use transverse incisions. Vertical incisions are rarely used.

However, if a pregnant woman had a vertical incision for her first child, then for the second child, she should also make a vertical incision, because the wound should be cut along the previous incision mark, and it is not possible to make a horizontal incision this time, leaving a cross on the pregnant woman's abdomen.
